VERY IMPORTANT!!

Prior to participation of any kind, it is required that the following paperwork be on file at Cosby High School!

VHSL Physical Form
Concussion Form
Sudden Cardiac Arrest Form

Heat Related Form
Athletics Insurance Form

If you are / have a returning athlete, new copies of the above forms will not be mandatory until after May 1st. If you are / have an incoming freshman that was involved in athletics this past year, the physical that was provided to their middle school will work! Cosby High School will just need a copy of it!

All athletes new to the Cosby Cheer program, please bring all paperwork with you and hand to Coach Sarah Kirwan at the first open gym you attend!
